The two release forms that each Vaulters of the Northwest Spring Fest participant [vaulter, coach, longeur, volunteer] must complete are now available. These forms are: 

Form M1, Permission for Medical Treatment & Form R1, Liability Release and Hold Harmless (click here to download)

Each club must submit completed and signed copies of each of these forms for each vaulter, coach and longeur, along with their entry fees by the close of entries on April 3nd, 2009.  Any missing or incomplete forms will result in a $5 late fee for each effected vaulter, coach and longeur.
